It is our desire to have a natural, un-medicated and intervention-free childbirth. We have educated ourselves and are prepared for the work involved. I understand that all of my requests presume that I have a normal pregnancy and birth. If there is a situation that constitutes a medical emergency for either me or my baby, I agree to accept medical judgments only with informed consent. I would prefer to have the following set of requests honored.

We greatly appreciate your cooperation in realizing our plan.

Environment:
~I prefer not to be separated from my birth partner during hospital stay.
~I would like to bring music, soft lighting.
~I prefer to have the lights dimmed.

Prep:
~I prefer no routine IV, if necessary, saline lock.
Labor and Delivery:
~I prefer no artificial rupture of membranes.
~I do not wish to be given pitocin.
~I do not wish to have medication offered to me, I will request if needed.
~I would like the freedom to walk and move during labor.
~I would prefer not to have constant fetal monitoring, I would like intermittent monitoring.
~I prefer no residents or students to be present during my birth.

Cesarean:
~In case of cesarean, I would like my partner to be allowed to accompany me to the OR and recovery room. I wish to be conscious, and would like to have immediate contact with the baby following birth (providing the baby is in good health).
~I would like my partner to be allowed to hold the baby and accompany him/her to the nursery.

Post-Birth:
~I plan on breastfeeding my baby, and would not like any pacifiers, or artificial anything be offered to my baby at any point.
